Thank you for submitting your information in the MLSE Employee Vaccination portal! Your submission indicates that you selected that you are exempt from vaccination.

MLSE will meet its duty to accommodate individuals under Code (Ontario Human Rights Code) protected grounds up to the point of undue hardship. Requests will be considered on a case-by-case basis for medical and or religious/creed reasons.

To proceed with the request, you are required to provide proof of exemption. Please contact vaccineaccommodations@mlse.com via email as soon as possible and attach the required supporting documentation. Failure to contact HR means that you will continue to be considered unvaccinated. You will also receive an email confirming receipt of your information at the e-mail address provided in your form.

If you requested exemption in error and instead wanted to submit your vaccine receipt, please simply resubmit your information through the form.
